CARES stands for the Community Access and Referral Enrollment System. This program provides discounted medical, dental, and mental health services based on income and family size for people who have no insurance. A growing list of health providers are participating in the CARES program, offering discounts on medical, dental, vision, and mental health services in La Plata County.

CHP+ and Medicaid
Family Medicaid is a public health insurance program for Colorado children age 0-18 and for parents with dependent children. The program is funded jointly by a federal-state partnership and is administered by the Department of Health Care Policy and Financing. Child Health Plan Plus (CHP+) is low-cost health insurance for Colorado's uninsured children and pregnant women. CHP+ is public health insurance for children and pregnant women who earn too much to qualify for Medicaid, but cannot afford private health insurance. Click here to go to the CHP Plus website.

Connect for Health Colorado
La Plata Family Centers will be providing Health Coverage guides with the following roles and responsibilities in navigating the new insurance marketplace: Provide customers with fair and impartial information and services that help guide them through the application process. Provide guidance to customers about their health plan options, and help them to prioritize, filter and sort these options. Assist in making changes to their account in the event of income fluctuations or changes in household, and renewing coverage during the appropriate renewal periods. Connect customers to the Customer Service Center, website or certified and licensed brokers if additional support beyond the scope of the Health Coverage Guide's role is needed.
